The health department addresses the health needs of both Ozaukee and Washington Counties by leading community coalitions, conducting a Community Health Improvement Plan, promoting health and well-being, collaborating with community partners, and addressing priorities at the individuals, family, environmental and policy level.

Community Health Assessments (CHA)
The Washington County and Ozaukee County Community Health Assessments are a compilation of health information collected from numerous sources to inform all of us about the health needs of our communities.

Community Health Improvement Plan (CHIP)
A community health improvement plan (or CHIP) is a long-term, systematic effort to address public health problems based on the results of community health assessment activities and the community health improvement process. The Washington County and Ozaukee County Community Health Improvement Plans will outline our commitment to keeping Washington and Ozaukee Counties ranking the top ten for overall health.

County Health Rankings
The annual County Health Rankings provide a revealing snapshot of how health is influenced by where we live, learn, work and play. They provide a starting point for change in communities.
