Community Health & Prevention

The Health Department addresses the health needs of both Ozaukee and Washington Counties by leading community coalitions, conducting a Community Health Improvement Plan, promoting health and well-being, collaborating with community partners, and addressing priorities at the individuals, family, environmental and policy level.

The Community Health Improvement Plan (CHIP) for 2022-2025 outlines our commitment to keeping Washington and Ozaukee County ranked in the top ten in the state for overall health as measured by County Health Rankings & Roadmaps (CHR&R), published by the University of Wisconsin Population Health Institute.

The annual County Health Rankings provide a revealing snapshot of how health is influenced by where we live, learn, work and play. They provide a starting point for change in communities.
